DATA WE COLLECT

Sedgwick collects information through the Site on behalf of AIG related to certain insurance products offered by AIG through Mastercard International Incorporated. Information collected may include without limitation personally identifiable information such as name, address, telephone number, e-mail address, social security number, account number and information related to medical history or treatment. Providing your personal information is optional however failure to provide the information requested may result in a delay in the processing of your claim/request.

USE OF COOKIES

When you visit the Site, we may also passively track information from your computer and Internet connection, such as the IP address of your computer and/or Internet service provider, the date and time you access the Site, the Internet address of websites from which you link to our Site, the computer technology you are using and your movements and preferences on our Site. Any insurance claim filed pursuant to your Mastercard sponsored insurance program on this website will be evaluated by AIG, Sedgwick, or their authorized local representatives and, if accepted, will be paid by AIG or by Sedgwick on behalf of AIG.
